Adult Case Management

Case Management services primary focus is to connect individuals to resources and other professional entities to assist in independence in the least restrictive environment.

An individual treatment plan is developed through an assessment process, member involvement, family involvement, and any other identified supports in place at time of intake.

The treatment domains may include the following:

  • Life Domain Functioning
  • Housing
  • Coordination of Care
  • Behavioral Health
  • Living Skills
  • Acculturation
  • Substance Use
  • Transportation Coordination
  • Educational

A Clinical review is provided by ABHS within the first 30 days of referral to assess the eligibility for the service based on risk criteria or a qualifying DSM diagnosis.

If the clinical criteria are not met for the service ABHS will provide the appropriate list of resources and assist with the referral to the next level of care.

Risk Criteria is limited to:

  • Homelessness
  • Psychiatric Hospitalization
  • Criminal Justice involvement
  • Mental Health PNMI residential placement

Case Managers are available to members during business hours and on-call for any crisis or emergencies that arise during non-working hours.
