Gain the Skills You Need to Live an Independent Life

Individualized program support developed through the comprehensive assessment of Life Skill Domains. Domains of support may include:

  • Developing and utilizing coping skills
  • Building on natural supports
  • Medication
  • Appointments (medical and psychiatric)
  • Community integration
  • Public transportation skills
  • Financial management
  • Routine development

These supports are provided 1:1 in the members home and community setting by a field staff trained as a Mental Health Rehabilitation Technician Level 1. Oversight is provided through administrative and clinical reviews.

Review of progress is completed through a team approach that involves the member, the members case manager, the support staff, the support staff supervisor, and any other individual the members wishes to have involved. The team approach continues to ensure that the member has a client centered plan with collaboration of care.

ABHS will provide the clinical assessment of need if the member or case manager is unable to provide within the first 30 days of referral. Qualifications of eligibility for the service are based on risk criteria or a qualifying DSM diagnosis.

Risk Criteria is limited to:

  • Homelessness
  • Psychiatric Hospitalization
  • Criminal Justice involvement
  • Mental Health PNMI residential placement

If the clinical criteria are not met for the service, ABHS will provide the appropriate list of resources and assist with the referral to the next level of care.
