Patient Navigator

The Patient Navigator is the primary provider of case management for all members in the Opioid Health Home.

The Patient Navigator shall be the primary provider of care coordination, health promotions, individual and family support services, and referral to community and social support services. Along with helping members with renewing MaineCare, housing applications and other forms of support. This roll works closer with all members of the Opioid Health Home team.

The candidate must have excellent communication skills, good computer skills, be motivated to make a change in our community and represent the agency and its mission in an engaging fashion.
